1. Not taking patient's own sign on the consent form,
2. Not taking procedure specific consent,
3. Not recording time of examination,
4. Not keeping copy of discharge ticket,
5. Not preserving OPD records,
6. Not taking proper precautions while going on vacations and appointing Locum doctor,
7. Not dealing with patients and their relatives properly after complications,
8. Not selecting patients judiciously,
9. Not demanding patients photo ID at the time of admission,
10. Not taking precautions while examining female patients,
11. Not having proper professional indemnity (prefer 1:1, minimum 50 lacs for physicians and 1 Cr. for surgeons),
12. Not stopping criticizing fellow colleagues and provoking patients to take action.
