Life After Lockdown

Emergencies do not announce themselves before happening. A Pandemic of this magnitude comes once in a century, if at all. It’s been a trying time for all of us and as human beings, we have sought to dealing with it in our own ways. For some it’s been difficult, being holed up inside with not a lot to do; whereas others have taken it as an opportunity of a life time to learn new things or develop hobbies they’ve always wished they could find time for.

Just chalking out how to function in a physically restrictive environment is a life skill in itself. Life, for a lot of us has reached a limbo or sorts. The only thing that is certain is that someday in the near future, this will end, and life will restart.
One question does arise though; what comes next?

Most will restart work, children looking forward to start going to school or college. Airports will open, travel and trade will resume. Things will slowly but steadily start heading towards normalcy.

Once that happens and “lockdown” becomes a distant memory, let’s not forget all the lessons this period has taught us. Let’s never forget that basic human compassion and tolerance towards each other is what gave us the strength to see a new dawn together. Literally and figuratively.

Personally, as a doctor I learned how to handle the most testing patients with a new level of patience. Living in what can resemble a dystopic novel can spook the best of us. A simple cold and cough become sinister when you don’t really know too much about your invisible enemy. I realized that sometimes people just need a few comforting words with Paracetamol to heal themselves.

We relearned the importance of a family unit, to thrive together. We learned about the virtues of living a disciplined life, when just waking up every morning, healthy with a roof on your head and food on your table was a privilege.

This period taught us patience, where the option of running the rat race ceased to exist and solitude and self-preservation became priority. We learned to enjoy our own company again. We bonded together as a community and helped who ever we could. We recognized our potential as caregivers, even if we could affect the lives of only those in our immediate vicinity. 
Most importantly we never let go of our killer instinct, that together we will defeat this virus.

We are a country of 130 million people. Imagine if we continue to work on ourselves with all the above ammunition we’re now armed with, the heights we’ll be able to scale.

Our Government has been exceptionally good in handling this pandemic so far. Even with limited resources, timely decisions and regular revisions of advisories circulated by the authorities have paid huge dividends.

We are better placed today than most developed nations and I am certain we will continue improving. Industrialists are working to develop ventilators on war footing. Pharma companies are working round the clock and providing essential medicines.
The front runners of this battle; Doctors, nurses and all other health care workers are risking it all and working overtime to save lives. The police is working 24x7 to keep residents safe. Essential service providers, sanitation workers, delivery boys, are all taking care of our daily needs and making sure our lives don’t come to a grinding halt. The wealthy have come forward and donated handsomely to Covid19 relief funds.

As citizens of India, It is a proud moment, that today our country after taking care of our own requirement, is supplying Hydroxychloroquine (a Covid19 medicine) to 55 other countries and is also shipping out large batches of Paracetamol to first world countries like the UK. Ours is also the first country that brought our stranded citizens back from across the globe.
Special mention must be made of our Prime Minister; he has always mentioned that we, the people of India, are its biggest strength. And in times like this, its heart warming seeing the nation unite and soldier on to a healthy, happy new start.
I strongly believe that together we can make India a front-runner in the months and years to follow.

India has played its role responsibly. After Covid19 the world will look for an alternative to China when it comes to manufacturing or trade. Along with that we have a plethora of experts in other fields too. Some of the sharpest minds in medicine and IT come from our motherland.

Let’s take a pledge. I will help someone to develop a skill where I can. You can do the same. We will work an extra hour and give back a little more than we generally would. We will not hesitate to extend a helping hand to those that need it. We will stop ourselves from judging each other on the basis of colour, caste or creed. We will show kindness, to those around us and most importantly, to ourselves. We will be more focused in our approach. We will appreciate nature and all its gifts that we took for granted. We will plant more trees and reduce, re-use and recycle whenever we can.

By staying apart right now, we will together take our country to the next level, the new India. “Nothing is impossible”.

India fights corona

Hello  Good morning
I’m not a bhakt or believe in Blind Faith
just a normal Indian American

But I’ve been reading
every expert
from every country
All warning of global catastrophe when corona did come to india
When it came India
controlled  all borders
Like no other
They said
all fake measures
All are fake reports
India must have lakhs of cases
India isn’t doing testing etc
India is not Reporting
Well it’s been 2 months
since WHOs warnings
India has placed
the strictest controls
of any nation on earth
(Like China)
That is acknowledged
by WHO & UN & Economist
India’s rates 100 along on the measures index
where countries like US etc are at 70
India still have the lowest numbers of infection per million across earth
Some may argue, india don’t test enough, no Resources  , indians grew up in infection so immune to corona no scientific proof
But in 14 days even those untested should have shown infection ? Or died ?
It’s now more than a month and india still have it under control (except for one big incident in delhi)

Look at  Mumbai
See how well they have fought this Corona

Look at BMC,
who would have imagined such precision in epidemic control
Roads r being swept,
washed & sanitised
Water is being treated
Waste is being disposed off
Buildings in lockdown
Areas in containment
They work silently,
whilst indians  are at home in lockdown

People laughed
at the idea of Taali bajao, candle light
Both in India & also foreigners,
but after India doing it ,
You have UK NYC Paris Italy
all doing similar things

People criticised India’s lockdown as stupid
And said look at Singapore
They have nothing
Yesterday Singapore was forced to bring in a one month lockdown as Corona is back

Spain is now doing a
3 week lockdown again
UK is doing a one month lockdown
Italy has been in lockdown
NYC will also be forced to do a lockdown the cities borders

Think of brave crew
Who are flying to bring back Indians
And take other people to their homes

A few days ago even Pakistan & Iran ATC praised  indian pilots & Airlines flights
Also spare a thought for our cabin catering staff & the BMC guys who dispose off bio waste / hazmat suits

Yesterday I watched Donald Trump in his press briefing saying “I called PM Modi,
because India makes a lot of Hydroxychloroquine for COVID treatment ,
They need a lot too for their billion-plus people, but ‘Mr Modi promised he will try to help USA”

A Bangalore doc is treating the British Royal family for Corona with Ayurveda and we laugh at our own culture & people

India has stepped upto the plate
Tata’s & Mahindra’s stopped making cars
Now inventing ventilators in their factories
Taj Sats catering stopped supplying Airlines
Now it gives daily meals to Hospital doctors free
Taj hotels has opened its doors for all doctors & nurses
Reliance gave it’s hospital for COVID & also equipped 7 hills for treatment
Vedanta is supplying lakhs of PPEs across India
A sanitary pad IIT startup has been making masks for free distribution
A microbrewery Meadery in pune is making sanitisers
Domestic Airlines have carried  more than 1500 tonnes of Medical Cargo gratis
Many more companies have done lots of things free
Sikh gurdwaras supplying lakhs of meals free daily
Even in NYC & London more than a lakh meals daily

That’s the level to which India & it’s essential services - Doctors Nurses Cop’s airline staff, Industries have risen to fight this new war

That is the spirit of our India
In my  Life never seen anything like this
And I’ve seen coups, 2 Gulf wars, SARS Ebola, 9-11, volcanoes etc

We are the Stalwarts group
So let’s not criticise anyone’s efforts,
So let’s as Indians at least praise the brave efforts of all our colleagues & also the workers, Air Indians, ATC, Airline workers, CISF, Police, Doctors, sanitary workers firemen, essential suppliers first responders
It's Amazing what Narendar Modi has done and shown to world
I salute him as  a pride Indian living in USA

The famous war of Umberkhind is mentioned in the Guinness Book of World Records:

Shivaji was a King of International fame. In the span of 30 yrs of his career he fought with only two Indian warriors. All the others were outsiders.

Shahista Khan, who feared Shivaji even in his dreams was King of Abu Taliban and Turkistan.
Behlol Khan Pathan, Sikandar Pathan, Chidar  Khan Pathan were all warrior sardars of Afghanistan. Diler Khan Pathan was the great warrior of Mangolia. All of them bit dust in front of Shivaji.

Sidhhi Jowhar and Salaba Khan were Iranian warriors, who got defeated by Shivaji.
Sidhhi Jowhar later planned a sea attack. In response Shivaji raised a navy, the first Indian Navy. But before accomplishing the task Shivaji left this world. (He was poisoned.)

Google *"Shivaji, the Management Guru."* It's a full subject in Boston University.

Yet, we Indians know so little about him..... what a pity.... atleast let us make our future generation to know about this great INDIAN-chatrapati

Why do some doctors charge so much ?

Why do some doctors charge so much ?
Why are some hospitals so expensive ?
Yeah Aamir Khan Rajinikanth should charge the same price as Dino Morea and Uday Chopra
And movie ticket should cost the same in PVR multiplex with a recliner in Mumbai and gallery seat in single screen in jumritalayya
Does it sound crass ?
But it is fact of life
It takes money to Built hospitals will state of art technology and facilities
Good Doctors are a rare species like movie stars
I takes 40-50 years to make a good doctor
And when you buy shares of a hospital you expect returns on it
Govt spends 1.2 percent of budget on healthcare
It is reducing it every year
World average is 5.3 percent
It has washed off its hands from primary health and tertiary Health
74 percent of Indian health care is private money
Most of the tertiary health is done by private players
There is no system like NHS in India
There is no universal health coverage
When u let off the field for private healthcare then don't except Charity from it
But having said that even the most expensive hospital in India is 10 times cheaper than USA or West
But iphone or a Merc  is more pricier in India than in West
Indian healthcare is cheap as our human resources are cheap
You can't employ a doctor by paying ₹40000 (600 dollars) in US
That is the minimum wages for a doctor in India
You can't hire a nurse for ₹15000 in U.K.
Our equipment comes from west
All our CT scans MRI Scans are imported
We pay same price as west in dollars and top of that we pay import duty
Then how we charge 1800 dollars for open heart surgery when it cost 100000 dollars in US
Cos our Doctors and nurses are paid less compared to west
Even the top most doctor in India gets less than an average doctor in NHS
We are happy when Google pays 1 crore to a fresh out of college IIM guy
But if a doctor buys a Merc after 20 years of hard work when his hair is greyed
We feel so upset about it and say look he is earning so much
It is Govt job to give healthcare to us
It is not job of private players or individuals
If Indian govt increases healthcare spend by 1 percent for next five years - we can provide 10 million jobs

Indian health care

The Drama and The Tragedy of Indian Healthcare

“One doesn’t need to be a doctor to start a hospital, just as one can start a hotel without knowing how to cook”.

Big businessmen and corporates who must invest their money in profitable ventures knew this very well. Up came gigantic buildings under the title ‘advanced / world class healthcare’, where much was invested in the infrastructure and technology, but the quality of doctors was kept to bare minimum necessary: junior most who are willing to accept the atrocious sharing conditions. These new ‘Money Making Machines’ picked up very well, because they gave the  rich class an illusion that they can buy health with money, but also attracted the middle and lower classes under the ever-coveted term ‘cashless’. The fear of illness and death was exploited with aggressive marketing. Many things  started to change after 1992.

When one invests in many hundred crores, the profits must match. To maintain, there must be continuous patient flow, admissions, tests, procedures and surgeries. Indian society is quite vulnerable to advertising as well as corruption. All the marketing strategies and skills were aggressively employed in that direction. To catch patients for paying procedures and surgeries, to develop a patient base, free camps were arranged in the interiors. 'Free' is a universally loved term, most people blindly fall for it.

The idiotic term ‘whole body check up’ was invented to attract the illiterate, ignorant and hyperanxious crowds. Unnecessary tests were suggested and done happily under the false sense of health security.  PROs and marketing teams of various hospitals started visiting private doctors for the obvious: commissions on admission / tests / surgery.

Patients with even simple illnesses that can be treated at primary healthcare center were taken to bigger cities now, and they proudly boasted how much land they sold to get the right treatment.
The perpetual inadequacy or absence of federal / state healthcare infrastructure was never questioned.
Those who had insurance cover started thinking that they now own all the hospitals and doctors. They started behaving as if they are obliging the doctor / hospital by choosing them. ‘Only a good outcome is must because I am paying money’ became a universal expectation, and thanklessness, threats and vandalism started with almost every bad outcome.

Applying the consumer protection act in India, with high illiteracy, poverty and political interference was the final straw. Doctors now could not treat patients based upon only clinical judgement, because the patient and the courts would demand proofs. So tests became necessary, to the point of finding a proof. Evidence based medicine defeated the need for clinical judgement to treat simple conditions without having to do the tests. Doctors and patients both started to suspect each other, and grew apart.

Still some really patriotic doctors, clinics, smaller nursing homes and hospitals continued what was essential for the society, giving excellent healthcare at the lowest cost, based upon their patient base generated over decades of faith and trust. The ‘Clinical Establishment Act” broke the very spine of such hospitals, because it made western standards blindly mandatory for Indian hospitals. More licensing, over 30 permissions (bribes everywhere), and so much paperwork and infrastructure investment, that it was now impossible to keep healthcare low cost. The only thing that was excluded from such westernization was costing. As the doctor is the only face of healthcare for the patient, the ultimate blame for higher costs was conveniently placed upon his/ her shoulder. All doctors turned villains now.

The hidden plans to eradicate small hospitals and private practitioners are being successful now, given the hardships involved in running one’s own hospital. Thousands of small hospitals / clinics have closed down. Doctors now must join corporate hospitals if they want to continue as specialists. They have no control over billing and hospital policies, and are thrown out if they question anything.  Most people do not know that a doctor’s payment is less than 10 percent of most hospital bills, a standard corporate protocol all over India, and if the doctor wants to help the patient, it is his own bill that he/ she will have to sacrifice.

Mediclaim (Medical Insurance) is still at a fraudulent stage in India, with many deserving patients being denied insurance, while many patients being allowed to misuse it. Mediclaim companies now have two fatal policies; to encourage the accreditation business, thereby making it difficult for smaller hospitals to survive, and to dictate lowest healthcare costs for hospitals and doctor’s fees, whereby their own profit margin stays high. In such a scenario, the corporates either must inflate the bills or cut down on the quality of healthcare provided.

The big corporates required specialists who worked at the lowest salaries and accepted all their conditions. Those who had passed out without much merits from less academic medical colleges, those who had lower confidence and / or experience became easily available.  Instead of increasing govt medical colleges and seats, private medical education was encouraged, because the highest in the land prifited from that policy. The society never objected, because the dream of ‘buying a medical seat and ability of becoming a doctor even without merit’  had become a reality. Only a few exceptional students benefit from private medical education.

To pay for electricity and water and land and building with commercial rates, to get no subsidies, to pay full taxes, and then to be told to “subsidise”  patient bills for charity (whereas the govt hospitals keep on increasing it regularly), is atrocious. At every stage a hospital / doctor must bribe for permissions. Very few exceptions. In fact, people expect doctors to pay higher bribes.  Doctors belong to the same society as you, and mirror the same good and bad that the society has, there cannot be one-way blame game here. This does not mean there weren't greedy doctors, in fact the corporate policies helped some such doctors  flourish too.

There indeed were days when doctors earned too much money, some with good and some with deceptive means. Gone are those days. Now in each specialty there are a few who do well, and some who don’t. Most doctors struggle to maintain the incomes to continue offering good healthcare. Many have quit the profession. 

Even patients are mostly not happy with only honest and good treatment. They want multiple facilities and luxuries,  trained staff, accreditations, cashless facility, no marketing/ referral/ cut practice, but still the lowest rates. How will smaller hospitals compete in such a scenario with the giant corporates?

In a few months / years, small hospitals and private medical practice will have been completely eradicated, the only healthcare options will be government or corporate. After defaming the entire medical profession and creating wide rifts between the society and doctors, the marketeers of health and life corporates and insurance companies, will  keep on freely filling their coffers while providing compromised healthcare at all stages.

Indian studen in America

TOO GOOD!

It was the first day of school in the USA and a new Indian student named Chandra Subramanian entered the 4th grade.

The teacher said, "Let's begin by reviewing some American History.

Teacher :- Who said 'Give me Liberty, or give me Death'?"

She saw a sea of blank faces except for Chandra, who had his hand up:

Chandra :- 'Patrick Henry, 1775,'.

'Very good!

Teacher :- Who said 'Government of the People, by the People, for the People,
shall not perish from the Earth?''

Again, no response except from Chandra:- 'Abraham Lincoln, 1863'.

The teacher snapped at the class, 'Class, you should be ashamed.
Chandra, who is new to our country, knows more about our history than you do.'

She heard a loud stage whisper: 'Fuck the Indians.'

'Who said that?' she demanded. But Chandra put his hand up. 'General Custer, 1862.'

At that point, a student in the back said, 'I'm gonna puke.'

The teacher glares around and asks 'All right! Now, who said that?'

Again, Chandra says, 'George Bush to the Japanese Prime Minister, 1991.'

Now furious, another student yells, 'Oh yeah? Suck this!'

Chandra jumps out of his chair waving his hand and shouts to the teacher,
'Bill Clinton to Monica Lewinsky, 1997.'

Now with almost mob hysteria someone said
'You little shit. If you say anything else, I'll kill you.'

Chandra frantically yells at the top of his voice, 'Michael Jackson to the child witness testifying against him in 2004.'

The teacher fainted. And as the class gathered around the teacher on the floor, someone said, 'Oh shit, we're screwed!'

And Chandra said quietly, 'Lehmann Brothers, November 4th, 2008'

Indian husbands

*Why Indian Husbands remain faithful disciples of their wives ?*.

I will tell you my personal story as to why I remain faithful to my wife. It is just for survival.

Her multitasking always amazed me, especially when it came to chemistry. It was 8 in the morning.

In the kitchen, my wife was checking the viscosity of the sambar, the solubility of chutney and the permeability of coffee, all at the same time.

The huge number of multisized, multilabel bottles and cans on the shelf in front of her, looked like a 17 th century alchemist lab with containers of different shapes and sizes labeled in Hebrew and Arabic. Hebrew and Arabic you can learn, but here it was a different challenge.

The ‘Horlicks’ bottle contained chili powder,
The ‘Bournvita’ tin, salt, while the oats tin had turmeric.
I won’t be surprised if the rat poison cover had pickles in it. But even without the blink of an eye she confidently opens a container labeled Britannia biscuit , puts a little masala powder in one of the cookers, and put it off. No watches to time, no weighing scale to go by.

But then suddenly, there was this call from the department of physics.
The milkman has come with the milk. Even without a lactometer she predicted that the milk did not meet the required specific gravity to be deemed fit for human consumption, adult, pediatric and our pet cat included.

The simultaneous arrival of the old newspaperwalla needed supervision in weights and measures department, which she did with accuracy eligible to earn an ISO certification.

Within the next 20 minutes she noted that the particulate matter in the tap water was alarmingly high (to me it seemed usual) and made sure that no one used it for drinking.

By noon she discovered an unusual biologic phenomena of slowing of the movement of our goldfish in our jerry can sized aquarium and predicted that the prognosis and outcome was poor. With my postgraduate degree in medicine, I did not find the fish too abnormal but uttered an “yes yes”, more to avoid an argument than anything else. By evening 6 the goldfish died. She followed the exact norms of environmental safety in disposal of the mortal remains with necessary prayers; identifying that the cause for the mortality was the new brand of fish food, without an autopsy.

The arrival of the gardeners made her take critical decisions in the area of agriculture deciding on which manure would suit the mango tree better.

And the financial wizard happily manages to run the home with a fiscal deficit starting by the end of third week of every month.

It is said that management skills go beyond what you are taught in the B schools., However top grade they are, you need some special senses. I agree. Even if my salary check lies deep in my hospital coat pocket she smells it out by the time I park the car.

Phenomenal I say. Isn’t it. Surprising that with all these qualifications, a working day from 6 am till 11 pm, and on call 24 hours a day, Indian housewives are still considered unemployed and have no organized Trade Union.

Administering the department of Home, with two terrorizing kids is another matter I don’t want to discuss here, because, at least one of them is a young ‘Activist’.

Today is the era of re-designation, where the department heads have become Directors, Managers rechristened as Chairman and owners as CEOs.

Without call for a strike, we have decided to elevate and promote the good old, young at heart, all knowing housewife to a "Homemaker."

She is happy with it!

I have very little knowledge of chemistry, nature science, food and beverage or commerce or arts.

I remain faithful to my wife, and I cling to her just for survival.
